The Freedom Of Information Privacy Act requires consent from parents/guardians of minors in the province of Alberta to allow us to share your child's name, birthdate and jersey number across associations for the purposes of games, tournaments and other publications such as Teamsnap. Ponoka Minor Hockey Association has also initiated a feature page in our newsletter which may feature your child, along with a picture and a short interview about their love of hockey and other interests. The Association also has a Facebook page where pictures of hockey action may be posted from time to time and is also fed to our webpage www.ponokaminorhockey.com. Please complete the following form so that we know which children may be featured under the Act with your permission.
